We believe in patient-centered health care and have attained official certification from the State of Minnesota as a Health Care Home for each of our three clinic locations in Rogers, Plymouth, and Crystal Minnesota.
What is a Health Care Home?
A primary care clinic or clinician certified by the Minnesota Department of Health to coordinate care among the primary care team, specialists and community partners to ensure patient-centered whole person care and improve total health and well-being.
Patients living in the Twin Cities of Minneapolis and Saint Paul who are seeking a primary care physician, doctor to delivery their baby, urgent care, or family doctor, can benefit from our top-ranked medical services at our clinics in Plymouth, Rogers, and Crystal, MN.

What is an independent clinic?
Simply put - an independent clinic is physician owned and operated. At Northwest Family Clinics, we take pride in our independent business model. The owners of our clinics are physicians. We work side by side with our dedicated staff to provide exceptional care to patients without the burden of heavy administrative overhead.
What is Team Care
Northwest Family Clinics provides collaborative team care that is centered on you.
Your care team is comprised of your primary care physician, your care coordinator, your provider’s medical assistant, clinic nurses, the clinic support staff, and may include specialists, pharmacists, and a family member, if you choose. The main advantage of this approach to medical care is that your care is comprehensive, coordinated, and delivered by a team of health care professionals who know you. These individuals are in frequent communication and have your well-being as their number one priority.
Everyone involved in your care is knowledgeable about all aspects of your health including: medical history, medical conditions and short and long-term goals. This builds strong, long-lasting and trusting relationships with providers and staff.
