Even more alarming, the World Obesity Federation released a report in March 2023 stating that by 2035, over 4 billion people – more than half the world's population – will be obese. This projection is staggering, especially considering the already high rates of heart disease, stroke, cancer, and type 2 diabetes, all of which are linked to obesity-related conditions.

Awareness is key to begin preventing and controlling this rampant problem. As a group of family physicians we’d like to take some time to explain obesity, its causes, and associated risks is crucial for public health awareness. Let’s demystify obesity and the importance of addressing this growing concern.

What is Obesity?

Obesity is a condition characterized by excess body fat accumulation to the extent that it may have a negative effect on health. It is typically measured using the Body Mass Index (BMI), where a BMI of 30 or above is classified as obese. However, it's important to note that BMI is a rough estimate and does not differentiate between fat and muscle mass. BMI is calculated by taking body weight divided by height, it therefore is not a perfect science as special circumstances like bodybuilders would likely fall into the obese category based on this simple calculation.

The standard BMI categories are:

  • Underweight: BMI less than 18.5
  • Normal weight: BMI 18.5 to 24.9
  • Overweight: BMI 25 to 29.9
  • Obesity: BMI 30 or higher

Causes of Obesity

The causes of obesity are multifaceted and include a combination of genetic, behavioral, metabolic, and hormonal influences. Key factors contributing to obesity are:

  • Dietary Habits: High intake of calories, especially from processed and fast foods high in fat and sugar.

  • Environmental Factors: Lack of access to healthy foods, high-cost healthy options, and limited safe spaces for physical activity.

  • Certain Cancers: Obesity may increase the risk of certain types of cancer, including breast, colon, and endometrial cancers.
  • Osteoarthritis: Extra weight can put more pressure on joints and cartilage, causing them to wear away.
  • Sleep Apnea and Respiratory Problems: Excess fat around the neck can block the airway and cause sleep apnea.
  • Gastroesophageal Reflux Disease (GERD): Obesity is linked with a higher risk of GERD, which can lead to heartburn and other complications of the esophagus.
  • Mental Health Issues: Obesity can affect mental health, leading to issues like depression, anxiety, and low self-esteem.
